A Choice: Poverty or Prosperity

"I hated poverty. Poverty is more of a choice than anything else, and I can change that (poverty). It depends upon how hard I want to work, and I decided that I would change it.

What he's trying to say here is that he hated living in poverty. he thought he was born into it because he was a bad person. At first, he was the worst student in school with the worst grades. He started working hard and eventually became the best. He is now a surgeon. He worked hard. Getting out of poverty was his motivation. If you work hard for what you want, you can do it. He new what he wanted to do. He didn't give up. He just pushed himself even more.
